Some of the data is self-reported by the facilities and is not independently verified by the Ohio Department of Aging. Consumers are encouraged to visit any facility they are considering.
Annual inspections of nursing homes and residential care facilities are performed by the Ohio Department of Health on a periodic basis (every 9 to 15 months). Conditions at a facility can change significantly between surveys. Please contact 614-752-9524.
Inspections of supportive living facilities and residential treatment facilities are performed by the Ohio Department of Mental Health and Addiction Services. Inspection reports are available by request; please contact 614-752-8880.
