Things to Remember About the Long-Term Care Consumer Guide 

There is no substitute for visiting any facility you are considering for yourself or your loved one.

Every effort is made to keep the information in the Long-Term Care Consumer Guide accurate and up to date. However, consumers are advised that it is possible for inaccuracies to occur. Some of the data is self-reported by the facilities and is not independently verified by the Ohio Department of Aging. 

Annual inspections of nursing homes and residential care facilities are performed by the Ohio Department of Health on a periodic basis (every 9 to 15 months). Conditions at a facility can change significantly between surveys, for the most updated information, please contact the Ohio Department of Health.

Inspections of Residential Facilities Class 2, also known as Adult Care Facilities or Group Homes, are performed by the Ohio Department of Mental Health and Addiction Services. To request an inspection report, please contact OhioMHAS.
