You have the right to expect excellence from the providers who give long-term care and support to your loved ones. Working with the Scripps Gerontology Center at Miami University, the Ohio Department of Aging is surveying family members, friends, or guardians of Ohioans who live in long-term care facilities, including nursing homes, residential care facilities, and hospital sub-acute units. We want to know what you think about your relative’s or friend’s home. The results of this survey will be added to the Long-Term Care Consumer Guide in 2022.
Individuals selected to complete this survey received a notification packet in the mail. They can submit their survey responses online using the instructions on the front cover of the survey packet, or they can opt to complete the printed survey in the packet and return it using the included postage-paid envelope. Survey responses should be submitted within two weeks of receiving the survey packet.
If you have concerns about the care your loved one is receiving, please contact the Office of the State Long-term Care Ombudsman at 1-800-282-1206 or complete our online complaint form for assistance. Comments written on the Family Satisfaction Survey form may not be seen by an ombudsman who can help.

The surveys below are samples for your review. Families will receive the surveys directly through the mail between September and December 2021. Family addresses are obtained from the assisted living or nursing home. Please check that your loved one's long-term care facility has your current mailing address to ensure you receive a survey and are able to participate.
